Trial Class Waiver - Fill this form out and bring it to the gym if your child will be participating in a trial class. Student Registration - These forms are required for student enrollment into a class. Open Gym Registration - If you are planning to attend our open gym please download and fill out these forms. Drop Card - This form is required if you wish to disenroll your child from a class part way through the session. Party Registration - Download, fill out these forms, and bring them to the front desk when booking your birthday party. Camp Registration - Download, fill out this form, and bring it to the front desk when you enroll your child in Summer Camp.
